Additional Notes. This company name change process is still in progress. My nonprofit has a 501c3 status as Padma Sherni Inc. 47-1256775.
We also filed our 990 tax forms as Real Health Inc. and wanted to use two dba names: (1) Padma Sherni Inc. and (2) Health and Education Research Services Inc. The document filed however has wrong information. The request was actually written first as the following: "Hi, I wrote my certificate of amendment document with the business name change request however the dba name is Padma Sherni, Inc. and the new name should be Real Health, Inc. FEIN 47-1256775 The document that is spelled with an acronym abbreviation that stands for Research, Evaluation, Advocacy, and Learning company. (1) On any updated form, can you please send me an updated business license with Real Health Inc. for the name indicating the dba Padma Sherni, Inc.? (2) Please indicate this request on the certificate of amendment. I have already seen the check for $194 cashed. Thank you, Pialee Roy, Ph.D." |
